At the Mix and Melt area, located at 2121 Huron Street, in Jacksonville, Florida: On or about June 14, 2023, and at times prior, the employer exposed employees to caught in and struck by hazards, in that the employer failed to take the Loader (Waldon) out of service and allowed employees to continue operating the power industrial vehicles with safety deficiencies such as but not limited to: inoperative horn, inoperative lights, bad controls, hydraulic leaks and deteriorate cushion seat.
