Capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly: Employees engaged in forklift operations at Bish's RV of Lincoln located at 1000 Saltillo Road and 1040 Saltillo Road, Roca, Nebraska, were exposed to crush and struck-by hazards in that forklift tow hitch attachments were not pre-approved by the manufacturer prior to use. Employees were using the forklift tow hitch attachments to move RV's in and around service drives, maintenance bays and the property.
