The employer must provide, and ensure each employee uses, a safe means of access and egress to and from walking-working surfaces:  Employees engaged in administrative activities at Bish's RV of Lincoln located at 1000 Saltillo Road, Roca, Nebraska, were exposed to trip and fall hazards in that employees defeat the rolling ladder guardrail to access the mezzanine storage, which is greater than 9 feet to the next lower level.

Capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly:  Employees engaged in forklift operations at Bish's RV of Lincoln located at 1000 Saltillo Road and 1040 Saltillo Road, Roca, Nebraska, were exposed to crush and struck-by hazards in that forklift tow hitch attachments were not pre-approved by the manufacturer prior to use.  Employees were using the forklift tow hitch attachments to move RV's in and around service drives, maintenance bays and the property.
