29 CFR 1910.28(b)(1)(i): Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i):  On or about April 1, 2025, it was identified that employees engaged in recreational vehicle servicing and maintenance activities at BRV Lincoln, LLC dba Bish's RV of Lincoln located at 1040 Saltillo Road, Roca, Nebraska were exposed to fall hazard in that employees working indoors atop recreational vehicles between 8 to 14 feet high were not protected from falls greater than 4 feet to a lower level.  Instances include but are not limited to an employee working for 3 hours on top of an RV re-roofing with new materials, repairing the air conditioning and vents.

29 CFR  1910.178(l)(5): The employer did not evaluate the competency of the operator who had previously received training appropriate to the truck and working conditions encountered.   On or about April 9, 2025, it was identified that employees performing recreational vehicle servicing and maintenance at BRV Lincoln, LLC located at 1000 and 1040 Saltillo Road, Roca, Nebraska were exposed to struck-by hazards in that the employer did not conduct an evaluation of all operators' performance prior to allowing operation.  Instance include an operator with past undocumented experience from a previous company not having been given the hands on evaluation at this facility prior to operating the forklift.

29 CFR  1910.334(a)(2)(ii): There was a defect or evidence of damage that could have exposed an employee to injury and the defective or damaged item was not removed from service and an employee was using it before repairs and tests necessary to render the equipment safe were made:   On or about April 9, 2025, it was observed that employees performing recreational vehicle servicing at BRV Lincoln, LLC doing business as Bish's RV Lincoln located at 1040 Saltillo Road, Roca, Nebraska were exposed to electrical shock and burn hazards in that employees used damaged extension cords to externally power recreational vehicles.  Instances of exposure included, but were not limited to:  i. The extension cord outer insulation was pulled back exposing the interior insulation to environmental conditions for which the interior insulation was not designed to protect against and  ii. The receptacle housing was cracked and abraded, typically evidence of being drug across the ground, thus diminishing the housing's structural integrity.

On or about April 9, 2025, it was documented that employees performing recreational vehicle servicing and maintenance in the service bays of BRV Lincoln, LLC dba Bish's RV of Lincoln located at 1000 and 1040 Saltillo Road, Roca, Nebraska were exposed to chemical burn/irritation hazards in that the employer did maintain and make available a chemical inventory list which is reflected by each individual chemical safety data sheet. Instances of chemicals found in the facility but not identified in a list include but is not limited to:  i.      Clorox Bleach, ii.     Meguiar's Detailer Glass Cleaner,(concentrate),  iii.    Ultimar Exterior Spray on Protectant (concentrate), iv.     Dicor Rubber Roof coating System (sealants), v.      Propane, vi.    Ultimar Leather & Fabric Protector vii.
