29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system. (a) Top of second floor on back side of house: On June 15, 2023, and times prior, employees were installing rafters for a gable roof. Employees were not using fall protection and were exposed to falls up to 23 feet from the top of second floor to ground. (b) On the roof at the back side of the house: On June 15, 2023, and times prior, an employee was installing sheathing on the roof of a two story home. The employee was not using fall protection and was exposed to a fall of 23 feet eave to ground.

29 CFR 1926.1053(b)(4): Ladders were not used for the purpose for which they were designed. (a) Top of second floor on the back side of the house: On June 15, 2023, an employee used an 8 foot step ladder to install a gable frame on the second story of the structure. The employee straddled the top of the ladder using the bracing for support and sitting on the top cap.
