29 CFR  1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval:   At  19459 S. Amherst St. in Gardner, Kansas employees engaged in residential framing activities were exposed to fall hazards.  Employees were lifted on/working from a wooden work  platform attachment on the all terrain fork truck.  The work platform was not approved for use on the all terrain fork truck by the manufacturer.

29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   At the jobsite located at  19459 S. Amherst St. in Gardner, Kansas;  Employees engaged in residential framing activities, walking the top plates, were exposed to fall hazards of approximately 16 feet to the exterior ground below in that fall protection was not utilized.

Amherst St. in Gardner, Kansas; employees engaged in residential framing activities were exposed to injuries associated with fall hazards in that the employer had not provided the employees with training to recognize, evaluate, and control the hazards.

Amherst St. in Gardner, Kansas; Employees engaged in residential framing and ground work activities were exposed to fall hazards from stairways that were not provided with stairails.  Stairails were not provided in the following locations:  a)   In the basement of the structure, 6 riser stairway was not provided stairails. b)  Between the first and second levels, 8 riser stairway was not provided stairails. c)  Between the second and third levels, 8 riser stairway was not provided stairails.
