Instance #2: On or about June 15, 2023, the employer did not ensure that employees working from heights approximately 9 feet, 11 inches from the next lower level on a residential roof were protected by the use of fall protection.

Jobsite: Instance #1: On or about June 15, 2023, the employer did not ensure that an employee who entered the ground level area where there was a hazard of falling objects wore hard hat.  instance #2: On or about June 15, 2023, the employer did not ensure that employees working on a lower roof where there was a hazard of falling objects due to employees working on a higher roof above them, wore hard hats.
