29 CFR  1910.304(f)(1)(viii): The employer shall ensure that all circuit breakers used as switches are labelled "SWD".   On or about June 20, 2023, and times prior thereto, the employees turn on lights from the electrical panel boxes and circuits were not listed and marked "SWD".  a) In the cutting shop area, electrical panel box used to turn on and off lights,  b) In the welding shop area, electrical panel box used to turn on and off lights.

On or about June 20, 2023, and times prior thereto, where the duct line main switch panel was not locked out and/or tagout prior to the maintenance employee(s) performing maintenance on the duct line machine, exposing the employee caught-in hazards.

On or about June 20, 2023, and times prior thereto, knockouts were missing from energized electrical panel boxes exposing employees to electrical shock hazards.  a)  In the welding shop, the power box that energize the Scotchman Iron Work machine was missing 2 knockouts,  b)  The TDF machine power box was missing a knockout.

29 CFR  1910.253(b)(2)(iv): Valve protection caps, where cylinders were designed to accept caps, were not always in place, hand-tight, except when cylinders were in use or connected for use.  On or about June 20, 2023, and times prior thereto, on the southside of the welding shop area, an unused oxygen cylinder stored without the protective cap exposed employees to struck-by hazards in the event the bottle was knocked over.
