On or about November 28, 2023, and at times prior thereto, at the coil line machine, a relocatable power tap was plugged into a 3-way extension cord to energize equipment such as but not limited to a computer and computer screen and was not used according to listing and labeling electrical requirements, exposing employees to fire and electrical hazards.

On or about November 28, 2023, and at times prior thereto, in the kitchen, the employer did not ensure employees had clear access to the fire extinguisher exposing employees to fire and smoke hazards.
