29 CFR 1910.29(b)(10): The employer did not ensure that when guardrail systems were used at hoist areas, a removable guardrail section, consisting of a top rail and midrail, was placed across the access opening between guardrail sections when employees are not performing hoisting operations. The employer may use chains or gates instead of a removable guardrail section at hoist areas if the employer demonstrates the chains or gates provide a level of safety equivalent to guardrails: (a) Grinding Department - Employees working from the operator platform and hoist area for TMB03 and TMB04 were exposed to a fall hazard of approximately five and a half (5.5) feet in that chain system did not protect to an adequate height (34.5 inches of a required 42+/-3 inches) and was lacking guarding on the west end of the platform, on or about June 22, 2023. (b) Grinding Department - Employees working from the operator platform and hoist area for TMB03 and TMB04 were exposed to a fall hazard of approximately five and a half (5.5) feet in that the chain system did not cover the west end of the platform, on or about June 22, 2023 Certification Not Required.
