29 CFR 1910.176(a): Where mechanical handling equipment was used, sufficient safe clearances were not allowed for aisles, at loading docks, through doorways or wherever turns or passage had to be made: a) Distribution center outbound dock area - On or about June 23, 2023, and at times prior to, the employer exposed employees to trip and stuck-by hazards in that, the employer failed to ensure that sufficient safe clearances were allowed for aisles at outbound dock area when employees were working with the boxes and loading the trucks.

29 CFR 1910.176(b): Material stored in tiers was not stacked, blocked, interlocked or limited in height so that it was stable and secure against sliding and collapse: a) Distribution Center/ Stacking racks - On or about June 23, 2023, and at times prior to, the employer exposed employees to struck-by hazards in that, the employer failed to ensure that materials stored on racks were stacked securely, limited in height and were stable against collapse when employees were operating powered industrial trucks and stacking pallets on the racks within aisles.
