On or about June 16, 2023 an employee engaged in fire operations related to prescribed burns suffered carbon monoxide poisoning due to smoke inhalation. The employee was not required to wear a respirator when performing the task. The employee was hospitalized and the employer failed to report the hospitalization to OSHA within 24 hours.
