29 CFR 1926.501(b)(13): The employer did not ensure that employees engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):  (a)Chokan Exteriors, LLC at 310 Prairie Avenue, Tea, SD 57064: On or about and at times prior to June 26, 2023, the exposing employer did not ensure employees were properly anchored/tied off while performing roofing operations. Employees were working on a 9/12 pitched residential roof without an adequate fall protection system. This condition exposed employees to fall hazards greater than six feet.  Chokan Exteriors, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1630493, citation number 1, item number 2a and was affirmed as a final order on March 14, 2023, with respect to a workplace located at 5005 West Equestrian Place, Sioux Falls, SD 57106.
