29 CFR  1926.102(a)(1):The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  On or about June 26, 2023, and at times prior thereto, at least three (3) employees that were performing roofing work using pneumatic nail guns without eye protection. Exposing them to struck-by hazards.

29 CFR  1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:   On or about June 26, 2023, and at times prior thereto, employees were installing shingles at heights of 6 feet and greeter without the use of a fall protection system, exposing the employees to the hazard of falling from heights.

29 CFR  1926.1053(b)(1):Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder was used to gain access:  On or about June 26, 2023, the side-rails of the ladder that at least three (3) employees used to access/egress the roof did not extend at least 3-feet above the upper landing surface. Exposing employees to fall hazards in the following instances (a-c):   a) Two ladders on the front of the house facing the street. b) One ladder on the east side of the house. c) One ladder on the west side of the house.
