The program shall enable each employee to recognize hazards related to ladders and stairways and shall train each employee in the procedures to be followed to minimize these hazards.  a) On or about June 27, 2023, at the previously addressed inspection site, employees accessing upper-level landing surfaces using portable ladders with side rails that did not extend at least three feet above the upper landing surface(s) were not provided a training program addressing ladder safety.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.   a) On or about June 27, 2023, at the previously addressed inspection site, employees accessing the top of walls to install roof trusses used a portable ladder with side rails that did not extend at least three feet above the upper landing surface exposing the employees to falls.
