29 CFR  1926.21(b)(2):The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  a)	William Terenso Meza. On or about June 28, 2023 at the address of 4640 Talmadge Rd. Toledo OH 43623. The employer failed to instruct employees in the recognition and avoidance of hazards commonly present when performing roofing work. Without the benefit of this training, employees performing roofing work activities were not equipped to recognized fall hazards from heights of 25 feet when operating from a scaffold platform and working atop the structure.

On or about June 28, 2023 at the address of 4640 Talmadge Rd. Toledo OH 43623: The employer did not ensure that employees were protected from head injury hazards while working below the roof line as employees above actively remove/replace roofing materials. Employees operating in this condition were exposed to struck by injuries from falling/flying objects such as tools and roofing materials.

On or about June 28, 2023 at the address of 4640 Talmadge Rd. Toledo OH 43623: The employer did not ensure employees were protected from falls while performing roofing work activities (installing asphalt shingles) from a ladder jack scaffold. Employees operating from the scaffold platform were exposed to fall hazards of 20 feet or greater to the ground below.

The employer did not ensure employees were protected from falls while performing roofing work activities (installing asphalt shingles) atop a three-story multi-family residential structure.  Employees operating in this conditions were exposed to fall injuries from heights of  25 feet to the ground below.
