29 CFR 1926.21(b)(2):The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: a) William Terenso Meza. On or about June 28, 2023 at the address of 4640 Talmadge Rd. Toledo OH 43623. The employer failed to instruct employees in the recognition and avoidance of hazards commonly present when performing roofing work. Without the benefit of this training, employees performing roofing work activities were not equipped to recognized fall hazards from heights of 25 feet when operating from a scaffold platform and working atop the structure.
