29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): William Terenso Meza, on or about November 3, 2023 at the address of 1494 Woodward Ave, Lakewood, OH : The employer did not ensure employees were protected from falls while performing roofing work activities (installing asphalt shingles) atop a two-story, single family residential structure. Employees operating in this condition were exposed to fall injuries from heights of 20 feet to the ground below. William Terenso Meza was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1679785, citation number 1 , item number 4 and was affirmed as a final order on August 31, 2023, with respect to a workplace located at 4640 Talmadge Rd. Toledo, OH 43623.
