29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  (a) On or about June  28, 2023, and at times prior, the employer failed to ensure workers utilized guarding and safety devices.  The electronic safety devices (interlocks) that ensure that the Graphic Packaging International machine on the beer line does not operate while doors are open, were bypassed via the magnetic sensors being taped together.   This condition exposed employees to caught-in hazards.    (b)  On or about June  28, 2023, and at times prior, the employer failed to ensure workers remained safe while operating the Climax machine.   Workers climb on the machine  when cans fall to correct the situation, and are thereby bypassing guards and exposed to caught-in hazards.

29 CFR  1910.219(c)(2)(i):  All exposed part(s) of horizontal shafting seven (7) feet or less from floor or working platform were not protected by stationary casing(s) enclosing shafting completely or by trough(s) enclosing sides and top or sides and bottom of shafting:  (a) On or about June  28, 2023, and at times prior, the employer failed to ensure that driveshafts of the outfeed conveyor from the seltzer line filler was properly guarded. The guarding was removed from the driveshaft  providing power transmission to the conveyor assemblies.  This condition exposed employees to caught-in hazards.

29 CFR  1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  (a) On or about June  28, 2023, and at times prior, the employer failed to ensure equipment was used in accordance with labeling. The relocatable power tap (RPT, aka "power strip") being used in the fermentation area had a UL listed amperage draw limit of 15 amps. The RPT was providing power to Sanitaire "Dry Time Air Mover" with a draw of 30 amps and a fluid flowmeter. Evidence of previous arcing was present on RPT.  This condition exposed employees to shock and burn hazards.

This condition exposed employees to shock and burn hazards.

29 CFR  1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:  (a) On or about June  28, 2023, and at times prior, a relocatable power tap (RPT, aka power strip) had its insulation pulled away from the plug  placing strain directly on the conductors attached in the plug.  This condition exposed employees shock and burn hazards.
