29 CFR  1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, shall be provided to assist employees in mounting and dismounting the ladder. In no case shall the extension be such that ladder deflection under a load would, by itself, cause the ladder to slip off its support.   a.) 2103 Termon Avenue, Pittsburgh, PA 15212: On or about June 28, 2023, employees were using a portable ladder to access a roof on the second floor of a three story home. The ladder side rails did not extend at least 3 feet above the upper landing surface. Employees using the ladder to access the roof were exposed to a fall hazard hazard of approximately 25 feet to the ground level.

The employees were exposed  to fall hazards of  heights up to approximately 25 feet. The employees were observed working on the roof while their harnesses were not connected to lanyards. The employees were exposed to fall hazards of approximately 25 feet to the ground below.  The (SGM General Contracting ) was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b)(13), which was contained in OSHA inspection number 1633824, citation number 01, item number 01 and was affirmed as a final order on April 6, 2023, with respect to a workplace located at 527 & 529 East End Avenue Pittsburgh,  PA 15221.  The (SGM General Contracting) was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b)(13), which was contained in OSHA inspection number 1585692, citation number 01, item number 1-a and was affirmed as a final order on  May 24, 2022, with respect to a workplace located at 220 Edelweiss Drive, Wexford, PA 15090.
