29 CFR 1915.73(d):When employees were exposed to unguarded edges of decks, platforms, flats, or similar flat surfaces, more than 5 feet above a solid surface, the edges were not guarded by adequate guardrails meeting the requirements of 29 CFR 1915.71(j)(1) and (2). Boatyard: The gangway onto the floating dock did not have a standard midrail guard, creating a fall onto the lower deck and/or into the water.

29 CFR 1915.158(b)(2):On floating vessels under 200 feet in length, at least one 30-inch U.S. Coast Guard approved ring life buoy with line attached were not located at the gangway. Boatyard, dock: The employer did not provide a throw ring near where an employee was working on the Chasseur boat over water. The life ring was located 75 feet away from the boat.
