29 CFR 1926.416(a)(1): The employer did not ensure that employees were not able to work in such proximity to any part of an electric power circuit that the employee could contact the electric power circuit in the course of work, unless the employee was protected against electric shock by deenergizing the circuit and grounding it or by guarding it effectively by insulation or other means: a) Job site, 631 West New Castle Street, Zelionople, PA: On or about June 26, during LED light installation activities, the employer did not ensure that an employee was protected from contact with live electrical components in that an employee left power on to the electrical panel, removed the cover and proceeded to disconnect wires to a circuit breaker, exposing him to electrical shock and explosion type hazards.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and a lanyard was not attached to the boom or basket when working from an aerial lift: a) Job site, 631 West New Castle Street, Zelionople, PA: On or about June 28, during LED light wiring patching activities, the employer did not ensure that an employee wore appropriate fall protection systems/devices when raised up in a bucket truck. Note to paragraph (b)(2)(v): As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under 1926.502(e).
