Routh Packaging Co. on or about June 29, 2023, at the address of 4413 Bogart Rd, Sandusky, OH: The employer failed to ensure the rotating auger of the Gut Truck Auger was adequately guarded. Employees performing operation activities at or near the nip-points and rotating parts of the auger were exposed to caught in / crushing hazards. B) HK Cooperative DBA J.H. Routh Packaging Co. on or about June 29, 2023, at the address of 4413 Bogart Rd, Sandusky, OH: The employer failed to ensure the rotating auger of the Barrel Dumper Auger was adequately guarded. Employees performing operation activities at or near the nip-points and rotating parts of the auger were exposed to caught in / crushing hazards.

Employees tasked with using the tool without appropriate factory guarding was exposed to the rotating wheel during metal grinding/cutting operations.

Routh Packaging Co was previously cited for a violation of an equivalent Occupational Safety and Health Standard 1910.305(b)(3)(i), which was contained in OSHA inspection number 1635799, citation number 1, item number 1a and was affirmed as a final order on 03-01-2023, with respect to the workplace located at 4413 Bogart Rd, Sandusky, OH.
