29 CFR 1910.147(f)(1)(ii):   Testing and positioning of the machine, equipment or components where lockout or tagout devices were temporarily removed from the energy isolating device, the employee was not removed from the machine or equipment area in accordance with paragraph (e)(2) of this section:  a.  On or about June 22, 2023, employees were exposed to crushed-by and amputation hazards when engaged in service and maintenance activities, specifically while troubleshooting the energized Hoegger Bacon Belly Press, Model 120500, Serial No. 11-087.   The employer did not ensure that employees implemented the required alternative troubleshooting procedures of ensuring employees were removed from machine area,  thus exposing them to crushing and amputation hazards.
