29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: At the jobsite located at 7924 SE 6th Terrace Blue Springs, MO: Employees were exposed to fall hazards of approximately 9 feet. The ladder used to gain access to the roof was not extended three feet above the upper landing surface and was not secured at the top.

Employees were conducting roofing activities without the use of fall protection. Vega Contractors LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1625907, citation number 1, item number 4 and was affirmed as a final order on 11/04/2022, with respect to a workplace located at 801 SE Major Road Blue Springs, MO.
