29 CFR  1910.178(o)(1): 29 CFR 1910.178(o)(1): The employer did not ensure that only stable or safely arranged loads shall be handled. Caution shall be exercised when handling off-center loads which cannot be centered:  The employer did not ensure the Behlen Country 10? Galvanized Round Stock Tank (approx. 1,117 gal.) was adjusted and handled for stable or safely arranged loads with the Utilev forklift. Employees engaged in stocking, rearranging, separating the nest quantities of 3 tanks stack/bundle together were exposed to struck-by hazards where the heavy tank fell, knock employee to the ground and pinned employee's leg.

29 CFR  1910.1200(e)(1): The employer did not develop, implement, and maintain at each workplace, a written hazard communication program which described how the criteria specified in 29 CFR 1910.1200 paragraphs (f), (g), and (h) for labels and other forms of warning, safety data sheets would be met:  The employer is failing to protect employees from chemical exposure hazards associated with using cleaning products in the store. Employees using hazardous chemicals, including but not limited to: detergents, soaps, plastic and plexiglass cleaner, lotion spray, air duster, bleach, alcohol solution, defoamer, WD-40, propane, were exposed to dermal and respiratory hazards due to the employer's written hazard communication program not being updated in accordance with the revised Global Harmonized System (GHS) Hazardous Communications Program. The current program was approved and revised in 2014.
