29 CFR 1910.178(a)(5):The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered: (a)Bomgaars Supply, Inc. at 2210 Broadway Avenue, Yankton, SD 57078: On or about and at times prior to September 12, 2025, the employer did not ensure employees were protected from struck-by or tip over hazards in that 72-inch fork extensions were used on forklifts onsite without the truck being marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered. This condition exposed employees to struck-by and tip over hazards. (b)Bomgaars Supply, Inc. at 2210 Broadway Avenue, Yankton, SD 57078: On or about and at times prior to September 12, 2025, the employer did not ensure employees were protected from struck-by or tip over hazards in that Vestil Model FE CAS-5, 63-inch forks extensions, were used on forklifts onsite without the truck being marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered. This condition exposed employees to struck-by and tip over hazards.
