29 CFR  1926.102(a)(1):The employer did not  ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  67 Medway St., Milford, MA: On or about June30, 2023, multiple employees were observed using pneumatic nail guns to frame and roof a structure without wearing eye protection exposing them to eye injuries from flying debris.

Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  67 Medway St., Milford, MA: On or about June 30, 2023, an employee was engaged in framing activities, on this commercial property, more than 14  feet above the next lower level and were not protected from falling by guard rails, safety net systems or a personal arrest system.

29 CFR  1926.502(d)(16)(iii):Personal Fall arrest systems  were not rigged such that an employee can neither free fall more than 6 feet (1.8 m), nor contact any lower level:  67 Medway St., Milford, MA: On or about June 30, 2023, two employees working on different sections of this bank being constructed had their personal fall arrest systems rigged so that even without the system fully deploying, the employee would impact the lower level before their fall was arrested.
