29 CFR 1926.502(d)(15): Anchorage points used for attachment of personal fall arrest equipment were not  independent of any anchorage point being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee.  Location: 17 Stanton Street, Malden, MA;  On or about 07/11/2023, employees were allowed to utilize temporary roof anchors which were not installed per manufacturer instructions to support 5,000lbs per employee.   a) Anchorage point on mid section of residential roof near chimney was not secured per manufacturer instructions and had multiple missing fasteners.  b) Anchorage point on the front section of residential roof nearest the street/front of home was not secured per the manufacturer instructions and had multiple missing fasteners.

Location: 17 Stanton Street, Malden, MA;  On or about 07/11/2023, Employees were observed with the d-ring located near their lower back, waistline, and ribcage area with visible gaps between the d-ring and their back while working 22-feet above a lower level from a residential roof.

29 CFR 1926.502(d)(21): Personal fall arrest systems were not inspected prior to each use for wear, damage, and other deterioration, and/or defective components were not removed from service.  Location: 17 Stanton Street, Malden, MA;  On or about 07/11/2023, employees were allowed to utilize rope lifelines while working from a residential roof 22-feet above a lower level which exhibited signs of wear and deterioration and were not removed from service prior to starting work.
