29 CFR  1926.502(d)(6)(i): Snap hooks were connected directly to webbing, rope or wire rope.   Location: Jobsite at 233 Beach Rd, Salisbury, MA  On or about 8/15/2024, the vertical lifeline snap hook of a personal fall arrest system was wrapped around a railing post and attached directly onto its own rope.

29 CFR  1926.502(d)(17): The attachment point of the body belt was not located in the center of the wearer's back. The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head.  Location: Jobsite at 233 Beach Rd, Salisbury, MA  On or about 8/15/2024, employees were exposed to fall injuries while utilizing life lines connected directly to the front/waist positioning ring and not using D-rings located at the center of the back.  Espinoza Construction Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.502(d)(17), which was contained in OSHA inspection number 1682245, citation number 1, item number 1-b and was affirmed as a final order on 5/13/2024, with respect to a workplace located at 17 Stanton Street, Malden, MA.
