Located at 605 Diley Road, Pickerington, Ohio: On or about July 13, 2023, the employer did not ensure that employees were protected from falls with an approved fall protection system while performing commercial roofing activities from heights of up to 13 feet above the ground. Employees were using unapproved climbing harnesses and unanchored rope lifelines as personal fall arrest equipment.
