On or about July 13, 2023, at a worksite located at 771 Francis Drive, Heath, Ohio 43056: The employer did not ensure strain relief was provided to cords so that the pull was not directly transmitted to joints or terminal screws. Employees used a black/orange extension cord where the jacket of the cord separated from the strain relief of the cord, thereby exposing employees to a 119-volt electrical contact hazard. b. On or about July 13, 2023, at a worksite located at 771 Francis Drive, Heath, Ohio 43056: The employer did not ensure strain relief was provided to cords so that the pull was not directly transmitted to joints or terminal screws. Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. a. On or about July 13, 2023, at a worksite located at 771 Francis Drive, Heath, Ohio 43056: The employer did not ensure that employees engaged in residential construction activities 6 feet or more above lower level were protected from falls. Framers were working on the second story of a residential home at heights up to 11 feet 3 inches. The employees were not utilizing any form of fall protection, thereby being exposed to falls hazards of 11 feet 3 inches. b. On or about July 13, 2023, at a worksite located at 771 Francis Drive, Heath, Ohio 43056: The employer did not ensure that employees engaged in residential construction activities 6 feet or more above lower level were protected from falls. Framers were working on the second story of a residential home where the stairway opening to the basement floor was not guarded. The employees were not utilizing any form of fall protection, thereby being exposed to falls hazards of 18 feet.
