29 CFR  1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  a. On or about July 17, 2023, at the above addressed jobsite, employees conducting roofing operations,  fourteen (14') feet five (5") inches, above the ground, were not protected from falling.  The Slavon RC Inc. was previously cited for a violation of its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1394873, citation number 1, item number 1 and was affirmed as a final order on May 17, 2019, with respect to a workplace located at 1508 E. Waverly Dr. Arlington Heights, IL 60004.

29 CFR  1926.503(b)(1):The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  a. On or about July 17, 2023, at the above addressed jobsite, employees had been provided fall training, however, the employer failed to keep a written certification of record.
