OSH ACT of 1970 Section (5)(a)(1) The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: On or about June 28, 2023, at the worksite located at N3164 County Road Y, Oakfield, Wisconsin; an employee was exposed to electrical shock and fire hazards associated with an electrical cabinet which was modified and not used in accordance with the instructions included in the listing and labeling. While exercising a breaker installed within the modified cabinet the breaker failed and had an arc blast incident causing burn injuries to the employee.
