On or about 7/18/2023, the employer did not ensure employees working from the roof of a five (5) story building were protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i). One employee was on a boatswain's chair washing windows while another worker were assisting from the edge of the roof without being protected from falling.
