29 CFR  1926.501(b)(1):"Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a) Jobsite: 1827 S. 13th St. Philadelphia, PA 19148 - On or about July 19, 2023, the employer did not ensure employees conducting roofing activities from the roof of a commercial building, approximately 9 feet above the ground level, were protected from falls by the use of a fall protection system.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) Jobsite: 1827 S. 13th St. Philadelphia, PA 19148 - On or about July 19, 2023, the employer did not ensure that employees accessing the roof over the main entrance of the vacant commercial space on the first floor of the building, approximately 9 feet above the ground level, were provided with safe access by extending the extension ladder at least 3 feet above the walking/working surface of the roof.

29 CFR  1926.20(b)(1):It shall be the responsibility of the employer to initiate and maintain such programs as may be necessary to comply with this part.  a) Jobsite:  1827 S. 13th St. Philadelphia, PA 19148 - On or about July 19, 2023, the employer did not initiate or maintain an accident prevention program to address the associated hazards that employees are exposed to during roofing activities, such as but not limited to fall hazards.  Luis fiberglass LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1610295, citation number 1, item number 1-a, and was affirmed as a final order on January 19, 2023, with respect to a workplace located at 2656 Salmon St. Philadelphia, PA 19125.

The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.   a) Jobsite: 1827 S. 13th St. Philadelphia, PA 19148 - On or about July 19, 2023, the employer did not provide a training program for each employee conducting roofing operations from the roof over the main entrance of the vacant commercial space on the first floor of the building, and exposed to fall hazards of approximately 9', in the recognition and avoidance of fall hazards.  Luis Fiberglass LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number1610295, citation number 1, item number 1-b and was affirmed as a final order on January 19, 2023, with respect to a workplace located at 2656 Salmon Street Philadelphia, PA 19125.
