29 CFR 1960.8(a): The Agency head did not furnish employment and a place of employment to each employee that were free from recognized hazards that caused or were likely to cause death or serious physical harm. a) Denali Recycle Shop: On or about July 18, 2023, an electric hoist used to pick up recycled material did not have the rated load marked on the hoist or load block. This condition exposed employees to a struck-by and crushing hazard. b) Denali Recycle Shop: On or about July 18, 2023, an electric hoist used to pick up recycled material did not have any initial or periodic tests completed on the hoist block or hooks that were attached. This condition exposed employees to a struck-by and crushing hazard.
