This condition exposed employees to a struck-by and crushing hazard.   b) Denali Recycle Shop:  On or about July 18, 2023, an electric hoist used to pick up recycled material did not have any initial or periodic tests completed on the hoist block or hooks that were attached. This condition exposed employees to a struck-by and crushing hazard.
