The employer's procedures for summoning rescue and emergency services were to call the local fire department by dialing 911. The local fire department did not agree to act as the employer's permit-required confined space rescue service.   a)  On or about June 23, 2023, an employee entered the permit space of the Voss tunnel washer with noncompliant procedures for summoning rescue services and with no procedures for rescuing the entrant from the tunnel washer.  b)  On or about September 27, 2023, an employee entered the permit space of Girbau Dryer 1 with noncompliant procedures for summoning rescue services and with no procedures for rescuing the entrant from the dryer.
