29 CFR  1910.28(b)(1)(i):  Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i).  a) Wastewater treatment pond:  On or about July 19, 2023, a sanitary manhole at ground level was left open, exposing employees to a fall hazard of 4 feet.

29 CFR  1910.303(b)(1): Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees.  a) Wastewater treatment Plant:  On or about July 19, 2023, there was two electrical switches with exposed wiring outside of a facility with  no protection from the weather. This exposed employees to an electrocution hazard.

29 CFR  1910.303(g)(1):  Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment.  a) Wastewater Treatment Plant yard:  On or about July 19, 2023, there were two electrical panels (240 volts & 480 volts respectively) with a dumpster and a pile of wooden pallets stacked in front blocking access to the panels.  This exposed employees to a fire hazard and a delayed emergency response.

29 CFR  1910.303(h)(2)(v)(B):  Suitable guards were not provided for equipment exposed to physical damage from vehicular traffic.  a) Wastewater Treatment Plant yard entrance:  On or about July 19, 2023, there were two electrical transformers next to the drive-in entrance of the plant that did not have any barriers to protect it from vehicle traffic. This exposed employees to an electrical hazard.

29 CFR  1910.304(b)(3)(i):  All 125-volt, single-phase, 15- and 20-ampere receptacles installed in bathrooms or on rooftops were not equipped with ground-fault circuit-interrupter protected for personnel.   (a) Wastewater treatment office:  On or about July 19, 2023, an electrical outlet was installed in the bathroom with no GFCI installed, this exposed employees to an electrical hazard.

29 CFR  1910.305(j)(2)(vi):  A receptacle installed in a wet location where the product intended to be plugged into it was not to be attended while in use (for example, sprinkler system controllers, landscape lighting, and holiday lights) did not have an enclosure that was weatherproof with the attachment plug cap inserted or removed.  A) Wastewater treatment lab:  On or about July 19, 2023, an electrical outlet installed outside the lab building entrance had no cover to provide protection from the weather exposing employees to an electrical hazards.

29 CFR  1960.8(a): The Agency head did not furnish employment and a place of employment to each employee that were free from recognized hazards that caused or were likely to cause death or serious physical harm.  a) Wastewater treatment plant ponds:  On or about July 19, 2023, there were no vehicle curb or other effective barriers in place along the waterside edge of the sewage ponds. This condition exposes employees to hazards of driving vehicles into the water, drowning, and a biological health hazards.
