29 CFR 1960.8(a): The Agency head did not furnish employment and a place of employment to each employee that were free from recognized hazards that caused or were likely to cause death or serious physical harm: a) On or about July 19, 2023, a CM electric hoist used to pick up water pumps did not have the rated load marked on the hoist or load block. This condition exposed employees to a struck-by and crushing hazard. b) On or about July 19, 2023, a CM electric hoist used to pick up water pumps did not have any initial or periodic tests completed on the hoist block, chain or hooks that were attached. This condition exposed employees to a struck-by and crushing hazard.
