This condition exposed employees to a struck-by and crushing hazard.   b) On or about July 19, 2023, a CM electric hoist used to pick up water pumps did not have any initial or periodic tests completed on the hoist block, chain or hooks that were attached. This condition exposed employees to a struck-by and crushing hazard.
