29 CFR 1910.106(d)(5)(iii) Flammable storage was not kept in closed metal containers stored in a storage cabinet or in safety cans:  a) GYM LOCKER ROOM 163:  Fifty-four cardboard boxes, each containing 12 34-ounce bottles of hand sanitizer, a Class 2 flammable liquid, were stored on open shelves outside of a closed metal container or storage cabinet, exposing employees to fire hazards. b) GYM LOCKER STORAGE ROOM 165: Twenty-eight cardboard boxes, each containing 12 34-ounce bottles of hand sanitizer, a Class 2 flammable liquid, were stored on open shelves outside of a closed metal container or storage cabinet, exposing employees to fire hazards. c) SECURITY ROOM: Forty-five bottles of 67.6 ounces of hand sanitizer and 14 cases, each containing 12 19-ounce aerosol cans of Lysol, a a Class 2 flammable liquid, were stored on open shelves outside of a closed metal container or storage cabinet, exposing employees to fire hazards.

29 CFR  1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  a) KITCHEN: Employees who were exposed to corrosive chemicals during routine work, including Multi-Quat Sanitizer, did not have access to facilities for flushing of the eyes, exposing them to injury from contact. b) BLDG 643 PUMP HOUSE: Employees who were exposed to corrosive chemicals during routine work, including Dixichlor Max. did not have access to facilities for flushing of the eyes, exposing them to injury from contact. c) HAZARDOUS STORAGE: Employees who were exposed to corrosive chemicals during routine work, including Dixichlor Max., did not have access to facilities for flushing of the eyes, exposing them to injury from contact.

29 CFR  1910.134(k)(6):The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer:  MAINTENANCE SHOP:  Employees who wore half mask respirators and full face respirators voluntarily had not received a copy of the information contained in Appendix D.
