29 CFR  1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  At the job site located at 3124 N. Pine Grove Circle Wichita, KS, employees engaged in residential framing activities using pneumatic nail guns and cordless circular saws were exposed to struck-by hazards in that eye protection was not utilized.

Pine Grove Circle Wichita, KS, employees engaged in residential framing activities were exposed to fall hazards of approximately 12 feet from the eave of the residence to the ground below in that fall protection was not utilized.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  At the job site located at 3124 N. Pine Grove Circle Wichita, KS, employees engaged in residential framing activities from a 6:12 pitch roof were exposed to fall hazards of approximately 12 feet from the eave of the residence to the ground below in that the Gorilla ladder used to access the roof was not extended at least 3 feet above the eave or was not secured to the structure.
