29 CFR  1910.305(b)(2)(i): Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy:   (a)1415 West Rose Street, Walla Walla, Washington 99362, office facility, Heater/HVAC room, janitor supply room,  on or about July 20, 2023 and at times prior thereto, Employees were exposed to energized electrical circuit from an exposed electrical receptacle missing protective cover. resulting in serious injuries, disabilities and death from electrocution and fire hazards.

29 CFR  1910.242(b):Compressed air used for cleaning purposes was not reduced to less than 30 p.s.i.:  (a) 1415 West Rose Street, Walla Walla, Washington 99362, Warehouse facility, on or about July 20, 2023 and at times prior thereto, employees were exposed to serious injury due to compressed air used for cleaning of equipment and parts was not reduced to less than 30 PSI by a regulator, which could result in foreign material being blown in eyes causing serious injuries.  The Forest Service was previously cited for a repeat violation of this occupational safety and health standard, as contained in OSHA inspection number 126318, citation number 2, item number 4 and was affirmed as a final order on 03/28/2018, with respect to a workplace located at Barlow Ranger District, Ranger Station.

29 CFR  1910.157(e)(2):Portable fire extinguishers were not visually inspected at least monthly:  (a) 1415 West Rose Street, Walla Walla, Washington 99362, Warehouse facility, on or about July 20, 2023 and at times prior thereto, employees were exposed to potential fire hazard which could result in serious injuries or illness due to all portable fire extinguishers in the admin office building did not have  monthly maintenance inspections performed. Note:  Applies to any extinguishers provided in the workplace.  The Forest Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR  1910.157(e)(2), which was contained in OSHA inspection number 1594183, notice number 1, item number 1a and was affirmed as a final order on8/18/2022, with respect to a workplace located at Forest Service, Parkdale Work Center.  The Forest Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR  1910.157(e)(2), which was contained in OSHA inspection number 1261318, notice number 2, item number 6 and was affirmed as a final order on 3/20/2018, with respect to a workplace located at Mt. Hood National Forest.
