29 CFR 1910.29(b)(2)(i): The employer did not ensure that guardrails met the requirement that mid rails that are installed at a height midway between the top edge of the guardrail system and the walking-working surface. (a) 71 West Main Street, Pomeroy Washington 99347, Warehouse, Loft area, on or about July 20, 2023 and at times prior thereto, employees were exposed to fall hazard when stairs leading to loft area in warehouse facility were missing a mid-rail which would result in a fall from height of more than 4 feet causing serious injuries, disabilities, and death. The Forest Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.29(b)(i), which was contained in OSHA inspection number 1261318, notice number 2, item number 1 and was affirmed as a final order on 03/20/2018, with respect to a workplace located at Barlow Ranger District, Ranger Station.

29 CFR 1910.37(a)(4):Safeguard(s) designed to protect employees during an emergency (e.g., sprinkler systems, alarm systems, fire doors, exit lighting), were not in proper working order at all times: (a) 71 West Main Street, Pomeroy Washington 99347, PSE lounge, Recreation office, conference room, South basement area, on or about July 20, 2023 and at times prior thereto, employees were exposed to fall/trip hazards from exit route emergency light not operating properly which would impair employee vision in the event of an emergency, resulting in injuries, bruises, and minor disabilities.
