29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): 29 CFR 1910.178(l)(6): The employer did not ensure that each operator had been trained and evaluated as required by this paragraph (l): a. On or about July 26, 2023, at the warehouse area. Employees were exposed to struck by hazards when operating a Yale LPG-power industrial truck used to transport merchandise pallets without operator's certification of training and evaluation. NOTE: Certification of abatement is not required for this citation item as abatement was provided and documented by the compliance safety and health officer prior to citation issuance.
