29 CFR  1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware were not provided by the employer for isolating, securing or blocking of machines or equipment from energy sources.   On or about July 25, 2023 and preceding at the Military Ocean Terminal Concord C2 Alaska, LLC operations, employees were not provided with the proper equipment or hardware for isolating, securing or blocking of machines or equipment energy sources. Employees were exposed to electric, hydraulic, and kinetic energy hazards.

29 CFR  1910.147(c)(4)(ii): The procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance.  On or about July 25, 2023 and preceding at the Military Ocean Terminal Concord C2 Alaska, LLC operations, the employer was unable to provide or identify specific procedures for controlling hazardous energy.  Employees performing vehicle maintenance and repair did not have specific lock out/tag out procedures to effectively control all hazardous energy sources such as electric, hydraulic, and kinetic energy. Employees were exposed to electric, hydraulic, and kinetic energy hazards.

29 CFR  1910.184(c)(2):A sling was shortened with knots or bolts or other makeshift devices.  On or about July 25, 2023 a chain sling used to connect a drum handling attachment to the overhead crane in B-Bay was fastened into a loop using a bolt. Employees are exposed to crushing injuries from falling loads.

29 CFR  1910.184(e)(1):Sling identification. The alloy steel chain sling did not have permanently affixed durable identification stating size, grade, rated capacity, and reach.  In B-bay, the chain sling used to connect a barrel handling attachment to the south crane was not labeled with an identification tag. Employees were exposed to crushing injuries by not being able to identify its rated capacity.

29 CFR  1910.303(f)(2):Services, feeders, and branch circuits. Each service, feeder, and branch circuit, at its disconnecting means or overcurrent device, shall be legibly marked to indicate its purpose, unless located and arranged so the purpose is evident.  In B-bay, electrical panel L-2 did not have breakers labeled to identify the equipment they were used for. Employees regularly work in this bay and are exposed burns, smoke inhalation, and other injuries resulting from malfunctioning equipment.
