29 CFR  1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  Establishment - The following equipment lacked proper guarding to protect the operator from rotating parts, flying chips and the point of operation:  a. The Clausing 20 Drill press had no chuck guard. b. Two Bridgeport mills had no guards for the cutting head. c.  The Leblond Lathe had no chuck guard. d. The Kalamazoo belt sander  had insufficient guard to cover over the rotating shaft.

The employer failed to inspect an electrical power strip with damaged insulation in the cutting edge welding booth to power a grinder, fan, extension cord, lights prior to operation. b. The employer failed to inspect a light with damaged insulation on the power cord adjacent to yellow powder coat line prior to operation.

29 CFR  1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure.  Establishment - The employer used extension cords and power strips in place of permanent wiring specifically in the following locations:  a. In the P&A baking area, extension cords were attached to a power strip used to power an industrial sealer and a ventilation fan. b. In the compressor room, multiple extension cords  were used to power electrical monitoring equipment primarily flow rate sensors on the pipes. c. Located next to the parts storage by oven an extension cord was zip tied to raking for overhead lighting. d. In the Straight Plow fabrication area an extension cord was passed through a cage to power a second extension cord used to power a battery charger and third item.   e. In the R&D development area  a power strip was zip tied to column to power multiple pieces of equipment and lights on the bridgeport mill and LeBlond lathe.

29 CFR  1910.1200(h)(3): The employee training did not include the requirements of 29 CFR 1910.1200(h)(3)(i) through (h)(3)(iv):   Establishment - The hazard communication training for employees did not include all required measures including classifications of specific chemicals, procedures and appropriate work practices to protect themselves from hazardous chemicals used, and how to detect releases of the hazardous chemicals used.  Hazardous chemicals included but not limited to primers, paints, and corrosives (PPG Powder coat (PCTT39102), Bonderite phosphate replenisher and Sherwin William Industrial Enamel).
