29 CFR 1910.212(a)(3)(ii): Points of operation of machinery were not guarded or guards were not designed and constructed to prevent employees from having any part of their body in the danger zones during operating cycles: (a)Falcon Plastics, Inc. at 605 7th Street Southwest, Madison, SD 57042: On or about and at times prior to July 21, 2023, the employer did not ensure that the point of operation on the Ten-Sheeg Machinery Co. LTD Skiver, (model SM-618H) was properly guarded to prevent employees from having any part of their body in the danger zone during specific machine operations. The static machine guarding in place was insufficient to prevent access to the point of operation and resulted in an employee injury. This condition exposed employees to caught-in and amputation hazards. (a)Falcon Plastics, Inc. at 605 7th Street Southwest, Madison, SD 57042: On or about and at times prior to July 24, 2023, the employer did not ensure that the point of operation on the 18 Inch Mixing Mill (equipment #MM1801-3) was properly guarded to prevent employees from having any part of their body in the danger zone during machine operations. No guarding in place to prevent employees accessing the roller area. This hazard exposed employees to caught-in, crushing and amputation hazards.
