29 CFR 1910.1200(f)(10): The employer did not ensure that workplace labels or other forms of warnings were legible, in English and prominently displayed on the container or readily available in the work area throughout each work shift. (a) Umatilla Forest Service, Heppner Ranger District , 117 South Main Street, Heppner, Oregon 97836, gas house, on or about July 27, 2023 and at times prior thereto, employees were exposed to serious injuries from unlabeled container or labels that were not legible resulting to unknown chemical hazards which could cause serious burns or eye injuries or illnesses.

29 CFR 1910.151(c):Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: (a) Umatilla Forest Service, Heppner Ranger District , 117 South Main Street, Heppner, Oregon 97836, Office area and warehouse, on or about July 27, 2023 and at times prior thereto, employees were exposed to serious eye injuries from expired eye wash bottle solution located in the first aid kit, for First-Aid use in the event of a splash and emergency exposure to an irritant chemical to the eyes, which could aggravate the employee's condition, as the expired solution could be a source of a potential eye infection or irritation.
