29 CFR  1926.452(k)(2): All ladders used to support ladder jack scaffolds did not meet the requirements of subpart X of this part -- Stairways and Ladders.  373 Main St, Milford, MA:  On July 28, 2023, employees worked from ladder jack scaffolds that had support ladders with nearly twice the angle allowed by subpart X of this part.  29 CFR  1926.1053(b)(5)(i) required that non-self-supporting ladders be used at an angle such that the horizontal distance from the top support to the foot of the ladder is approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support).  The steep angle of the support ladders contributed to a scaffold collapse and multiple employees being injured.

29 CFR  1926.451(e)(1):  When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.   373 Main St, Milford, MA:  On July 28, 2023, employees climbed and descended the supporting ladders to access ladder jack scaffolds.  The scaffold platforms were more than 2 feet above the points of access and no other ladders were dedicated for accessing the scaffolds.

The lack of training and supervision resulted in scaffold erection deficiencies, a scaffold collapse, and multiple employees being injured.
