The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.  Location: 788 Main St, Hudson, MA,- Roof and on scaffolding  On October 25, 2024, employees were exposed to falls from heights up to 25 feet to the ground while conducting roofing activities where fall protection training was not provided to recognize and minimize fall hazards.

29 CFR  1926.451(e)(1):When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.    Location: 788 Main St, Hudson, MA- rear of house  On October 25, 2024, employees climbed and descended the supporting ladders to access ladder jack scaffolds.  The scaffold platforms were more than 2 feet above the points of access and no other ladders were dedicated for accessing the scaffolds.  SB Roofing & Construction was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 1686958, citation number 1, item number 2d and was affirmed as a final order on 10/31/2023, with respect to a workplace located at 373 Main St, Milford, MA.

SB Roofing & Construction, Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1662287, citation number 1, item number 1 and was affirmed as a final order on 10/4/2023, with respect to a workplace located at 518 Main St, West Newbury, MA.
