(b) Kill Room - On or about August 1, 2023, employees were exposed to electrical hazards in that extension cords were used as a substitute for the fixed wiring of a butcher saw. (b) Kill Room - On or about August 1, 2023, employees working in the Kill Room were exposed to electrical hazards in that the extension cord powering a butcher saw was not visually inspected before use on every shift for damage.
